FORTE VisionTree Manual

Once enrolled in FORTE, participants are asked to complete forms related to demographics, family history, health, diet, and lifestyle factors.

VisionTree emails often go to spam/junk, and can be difficult for participants to identify. Here are some options for site staff to assist participants with completing VisionTree PRO questionnaires:

  • After consent and randomization, send a sample email to the participant along with other instructions and information (such as the stool kit instructions) so they know to watch out for the nondescript VisionTree email  
  • Register another email address (such as your own) and forward the VisionTree email to the participant after randomization since they would be more familiar with your contact information  
  • Have the participant complete questionnaires on paper and manually enter the data into Rave  
  • Complete questionnaires over the phone and enter the data into Rave For detailed information and instructions, see Protocol Section 11.0. 

Review the FORTE VisionTree Manual on CTSU for further guidance on helping participants complete their questionnaires. For staff new to VisionTree or NRG-CC005, monthly training sessions with VisionTree are available for research professionals new to this tool.
